    What to bring with you

    Please bring a full print out of your medication and medical history from your GP. If you do not have this, you will need to ensure you have the NHS app connected to your GP so that we can access your medical information and medication list.

    If you do not provide this information on the day, we cannot carry out your assessment.

    A Group 2 driving medical assessment is a medical examination required by the DVLA for professional drivers, including HGV, LGV, PCV, taxi and private hire drivers.

    The assessment is used to confirm that a driver meets the DVLA Group 2 medical standards. The standards for HGV/LGV/PCV are stricter than those for private car drivers due to the size of the vehicle, passenger safety, and longer driving hours. The doctor completes the official DVLA D4 medical form/council specific form which you can submit to the DVLA.


    The frequency of driving medicals depends on your licence type and age. 

    For taxi, paramedic, PCV, HGV and LGV drivers, medical assessments are required at the following intervals:

    • At first application for a Group 2 licence
    • At age 45, then every 5 years until age 65
    • Annually from age 65 onwards

    You may need more frequent assessments if a relevant medical condition is present.


    The assessment is non-invasive and typically includes:

    • A review of your medical history and current medications
    • Vision testing, including eyesight standards required for Group 2 drivers
    • Blood pressure measurement
    • Assessment of cardiovascular health
    • Review of neurological and mental health conditions
    • Assessment of diabetes, if applicable
    • Discussion of alcohol or drug use, where relevant


    Following the assessment, the DVLA D4 medical form/council- specific form is completed accurately.


    The examination is non-invasive and usually takes 20–30 minutes. You will pay on the day.


    A driving medical assessment is required for professional drivers who hold or are applying for a Group 2 licence.

    This includes:

    • HGV and LGV drivers
    • Bus and coach drivers (PCV)
    • Taxi and private hire drivers (as required by local councils)
    • Paramedics / student paramedics
    • Drivers applying for, renewing, or maintaining a vocational licence

    Medical assessments are required at set age-based intervals and may also be requested by the DVLA or a local licensing authority if a relevant medical condition is present.

    Private car and motorcycle drivers (Group 1) do not require a Group 2 medical unless they are applying for a professional driving licence.


    Group 2 driving medical assessments are carried out by a GMC-registered doctor who is qualified to assess drivers against DVLA Group 2 medical standards.


    • Photo ID (such as a driving licence or passport)
    • Your glasses or contact lenses, if you wear them for driving
    • Details of any current medications
    • Relevant medical reports or letters, if you have ongoing conditions (for example diabetes, heart conditions, or sleep apnoea)
    • Your relevant D4 or council specific form to be signed by the medical practitioner. We can provide this for you if necessary.


    For taxi drivers/ private hire drivers:

    • Once you have successfully completed your medical assessment, you'll get your council specific form signed and stamped by the registered doctor, which you are then able to submit to the taxi council.

    For HGV/LGV/PCV:

    • Once you have successfully completed your medical assessment, you'll get your council specific form signed and stamped by the registered doctor, which you are then able to submit to the taxi council.

    For Paramedics:

    • The doctor completes the C1 medical paperwork or DVLA D4 form, depending on requirement
    • Your form is to be submitted to the DVLA or sent to the ambulance trust / occupational health department, depending on employer policy.
    • The DVLA or employer makes the final decision on your driving entitlement.


    If you do not meet the DVLA Group 2 medical standards at the time of your assessment, the doctor will explain the reason and what happens next.

    This may include:

    • Advising further medical evidence or specialist reports
    • Recommending treatment or review before reassessment
    • Completing the D4 form/council specific form with the relevant medical information for the DVLA or licensing authority to make a final decision

    The examining doctor does not make the licensing decision. The final outcome is determined by the DVLA or your local council, based on the medical information provided.

    In many cases, drivers are able to reapply or be reassessed once the issue has been addressed or stabilised.


    We require at least 24 hours’ notice if you need to cancel or reschedule your appointment.

    If you cancel within 24 hours of your appointment or fail to attend, you will be required to pay the full fee (£45). A letter will be sent requesting payment. 


    If you arrive more than 10 minutes late, we may not be able to proceed with your treatment, and this will be considered a missed appointment, requiring full payment.

    1. Introduction

    1.1 These Terms and Conditions govern the use of the Road2Health Ltd website and the provision of Group 2 driver medical assessments and related services.

    1.2 By accessing or using our website, you agree to be bound by these Terms and Conditions in full. If you do not agree with any part of these terms, you must not use our website.

    1.3 By booking a medical assessment with Road2Health Ltd, you enter into a contractual agreement and confirm your acceptance of these Terms and Conditions.

    2. Driver Medical Assessments

    2.1 It is the driver’s responsibility to ensure that the correct type of medical assessment has been booked for their licence or licensing authority requirements.

    2.2 If Road2Health Ltd is unable to carry out a booked medical assessment for any reason, our liability is limited to providing a full refund of the medical fee paid. We will not be liable for any consequential or indirect losses.

    2.3 A medical assessment cannot be completed if the required documentation or items are not provided. It is the driver’s responsibility to ensure they are aware of what is required. Any queries should be directed to head office on 0778 696 8324 or by email at admin@road2health.co.uk.

    2.4 Drivers must bring the following to their appointment. Failure to do so may result in the medical being unable to proceed, and no refund will be provided:

    • A list of current medications
    • Glasses or contact lenses (with the ability to remove contact lenses during the appointment)
    • Valid photo identification (e.g. passport or driving licence)
    • Relevant medical summary or records where required
    • The appropriate DVLA D4 form or council-specific medical form

    Any repeat medical required as a result will be charged at the standard rate.

    2.5 If a licensing authority subsequently refuses or delays a licence following submission of the medical, Road2Health Ltd accepts no responsibility for any resulting loss, including financial loss.

    2.6 If the examining doctor determines that you do not meet the medical standards set by the relevant authority (including, but not limited to, blood pressure readings exceeding 180/100 under Group 2 standards), the medical will be recorded accordingly. Doctors are required to adhere strictly to these standards and cannot amend or override guidance. Any repeat assessment will incur the standard fee.

    2.7 Drivers are responsible for ensuring that all information provided is accurate and complete. Road2Health Ltd is not responsible for forms rejected by the DVLA or a licensing authority due to inaccurate or incomplete information.

    2.8 If a licence is delayed or refused due to an administrative error on our part, we will arrange a corrected appointment where possible. If this is not feasible, our liability is limited to the cost of the medical assessment only. No additional losses or expenses may be claimed.

    2.9 Where amendments are requested by a licensing authority following submission, we will arrange a repeat assessment at no additional charge where possible. If not possible, a refund of the medical fee only will be issued.

    2.10 Drivers must provide honest, accurate, and complete medical information at all times.

    3. Cancellations, Non-Attendance and Late Arrivals

    3.1 Cancellations or appointment changes must be made with at least 24 hours’ notice. Requests made outside this timeframe will not be accepted and the full fee will remain payable.

    3.2 If a driver arrives 15 minutes or more late, the appointment may not be completed, subject to the doctor’s availability. A payment request will be issued to cover allocated time and resources. Unpaid amounts may be escalated and referred to a debt recovery agency.

    3.3 Failure to attend an appointment without at least 24 hours’ notice will result in a £45 non-attendance charge, which may be pursued if unpaid.

    4. Medical Personnel and Conduct

    4.1 All medical assessments are carried out by GMC-registered doctors in accordance with DVLA and licensing authority standards.

    4.2 Where a medical is successfully completed, documentation will normally be signed during the same appointment.

    4.3 Road2Health Ltd operates a strict anti-falsification policy. Any attempt to provide false or misleading information, interfere with testing, or tamper with results may be recorded and reported to relevant authorities.

    5. Privacy and Confidentiality

    5.1 All personal and medical data is handled confidentially and in accordance with UK GDPR and the Data Protection Act 2018.

    5.2 Information will only be shared with the driver, their authorised representative, or relevant authorities where legally required.
